FEV1 decline in cystic fibrosis.

چکیده
I read the paper by Taylor-Robinson et al with interest with regard to modelling the decline in forced expiratory volume in 1 second (FEV1) in patients with cystic fibrosis (CF). The authors have made the assumption that expressing spirometry results as a per cent of predicted (PP) is a valid way to use lung function over a wide age range. Young adult patients with cystic fibrosis die with absolute FEV1 values just as low as those found in older patients with chronic obstructive pulmonary disease (COPD), so PP is not likely to be a valid methodology. When young CF patients were transplanted on the basis of an adult-derived PP cut-off value, it was found that those operated on had a worse survival than those who were not, and this was due to the fact that in young subjects, a given PP value is at a higher absolute level than that in older subjects. We have shown, 4 as have others, that survival is better reflected by relating FEV1 to a power of height rather than to a predicted value. The authors should consider reworking their data with other ways of standardising their data to see if CF patients’ lung function can be better assessed. When comparing these alternative methods with the method of relating results to a predicted value, they should also consider using a single continuous prediction equation, such as the Lambda-Mu-Sigma equation that overcomes any hiatus in moving from paediatric to adult equations.

Positive Expiratory Pressure (PEP) versus Conventional Chest Physiotherapy in Pediatric Patients with Acute Exacerbation of Cystic Fibrosis
Background Pulmonary involvement is the main cause of mortality in cystic fibrosis (CF). Airway clearance techniques are non-pharmacological complement options for CF patients. The aim of this study was to evaluate the short-term outcome of airway cleaning treatment in patients with cystic fibrosis in a children's hospital. Materials and Methods This clinical trial study conducted on 40 CF pati...
متن کاملFlexible semiparametric joint modeling: an application to estimate individual lung function decline and risk of pulmonary exacerbations in cystic fibrosis
Background Epidemiologic surveillance of lung function is key to clinical care of individuals with cystic fibrosis, but lung function decline is nonlinear and often impacted by acute respiratory events known as pulmonary exacerbations. Statistical models are needed to simultaneously estimate lung function decline while providing risk estimates for the onset of pulmonary exacerbations, in order ...
متن کاملImpact of Long-Term Tiotropium Bromide Therapy on Annual Lung Function Decline in Adult Patients with Cystic Fibrosis
BACKGROUND Chronic lung disease is the leading cause of death in patients with Cystic Fibrosis (CF) and is often treated with bronchodilators. It is not known whether long-term tiotropium bromide treatment may have a positive impact on lung function. METHODS This retrospective cohort study estimated annual lung function decline utilizing longitudinal data for forced expiratory volume in 1 s (...
متن کاملAn observational study of matrix metalloproteinase (MMP)-9 in cystic fibrosis.
BACKGROUND In cystic fibrosis (CF), cross-sectional studies have reported sputum matrix metalloproteinase (MMP)-9 to be elevated and negatively correlated with FEV1. This longitudinal study examined the association between MMP-9 and tissue inhibitors of metalloproteinases (TIMPs) to prognostic parameters in CF. METHOD A cross-sectional survey of CF and control subjects; CF patients were follo...
متن کاملInvestigating the Effect of TNF α (-863) and TNF α (-308) genes Polymorphism on the Progression of Disease in Patients with Cystic Fibrosis
Background: Recent studies have shown that the course of cystic fibrosis in patients with this disease differs despite the same mutation in CFTR gene. We aimed to investigate the role of polymorphism in TNF α (-308) and TNF α (-863), and its effect on the phenotype of the patients with cystic fibrosis and progression of disease. Materials and Methods:...
متن کاملCYSTIC FIBROSIS Peak oxygen uptake and mortality in children with cystic fibrosis
Background: Single measurements of peak oxygen uptake (V̇O2) have been shown to predict mortality in patients with cystic fibrosis (CF) although no longitudinal study of serial measurements has been reported in children. A study was undertaken to determine whether the initial, final, or the rate of fall of forced expiratory volume in 1 second (FEV1) or peak V̇O2 was a better predictor of mortalit...
